<br />Cl'IY l'lLE tio. 5~ -q \ -~ <br /> <br />ADMINISTRATIVE ORDER <br />of the <br />CIlY MANAGER <br /> <br />FIRE SAFElY LICENSES ADMINISTRATIVE RULE R-3.175 <br /> <br />Pursuant to the authority contained in Section 2.019 of the Eugene Code, 1971, I <br />hereby adopt the following Fire Safety Licenses Administrative Rule R-3.175: <br /> <br />FIRE SAFElY LICENSES - ADMINISTRATIVE RULE R-3.175 <br /> <br />R-3.175-A. Application Information. <br /> <br />1. An applicant shall complete a fire safety license application form provided <br />by the City. <br /> <br />2. The application form may generally require the following information: <br /> <br />2.1 Name of the firm or organization; <br /> <br />2.2 Location of the facility; <br /> <br />2.3 Manager's and owner's name; <br /> <br />2.4 Mailing address and telephone number; <br /> <br />2.5 Nature of the facility's operation; <br /> <br />2.6 Type and amount of flammable liquids, explosive materials, <br />hazardous and/or toxic materials used, stored, dispensed, handled, <br />manufactured or disposed of at the facility. <br /> <br />3. A separate application form may be required for each facility. <br /> <br />4. If the applicant is a firm of foreign origin, additional information including <br />the place and date of incorporation, home office, right to do business within the state, firm <br />or organization officers and/or its duly authorized local agent may be required. <br /> <br />R-3.175-B. Fees Required of Applicant. <br /> <br />All application, processing, checklist review, special event, or other fees shall be <br />established by the City Manager pursuant to Section 2.020 of the Eugene Code, 1971. <br /> <br />Administrative Rule - 1 <br />
