Appendix B <br />Reporting Forms and Su Worksheet <br />The four pages in Appendix B contain examples of electronic reporting forms, and an example of <br />the certification form required to be submitted with materials balance reports, as well as a <br />worksheet that can be used to document chemical use. <br />Pages 34 and 35 show the electronic reporting forms as they will appear to businesses using the <br />reporting software supplied by the City.
