<br />EXHIBIT 11 <br /> <br />~ <br /> <br />REQUEST FOR DETERMINATION OF RELOCATION <br />COMPENSA TION <br /> <br />GENERAL INFORMATION <br /> <br />The following information provides a brief description of the process by which a <br />tenant or Owner/Developer (Applicant) may seek review of the decision of the <br />Housing Counselor regarding relocation compensation. For more information, <br />please refer to Section 2.1086(7) of the Eugene Code and to Administrative Rule <br />R-1086-G. <br /> <br />1. In the event that a dispute arises between the tenant and the Applicant over <br />the relocation compensation which cannot be mediated by the Housing <br />Counselor, either the Applicant or tenant may file a Request for Determination <br />of relocation compensation with the City of Eugene. <br /> <br />2. The City Manager shall allow the park owner, the tenant, and the housing <br />counselor the opportunity to submit written arguments, comments and <br />affidavits on the request. The determination of the City Manager shall be <br />final. <br /> <br />REQUIRED INFORMATION <br /> <br />The following information or material must be submitted with a Request for <br />Determination of Relocation Compensation: <br /> <br />1. Name of manufactured dwelling park to be closed: <br /> <br />2. Date of filing request for determination: <br /> <br />3. Name, address, and telephone number of affected tenant or Applicant with <br />whom the dispute is with: <br /> <br />4.
