<br />EXHIBIT 16 <br /> <br />. <br /> <br />REQUEST FOR DETERMINATION OF COMPARABLE HOUSING <br /> <br />GENERAL INFORMATION <br /> <br />The following information provides a brief description of the process by which a <br />tenant or Owner/Developer (Applicant) may seek review of the decision of the <br />Housing Counselor regarding comparable housing. For more information, please <br />refer to Section 2.1076(2) of the Eugene Code and to Administrative Rule <br />R-1078-G-2. <br /> <br />1. In the event that a dispute arises between the tenant and the Applicant over <br />the status of an offered unit as comparable which cannot be mediated by the <br />Housing Counselor, either the Applicant or tenant may file a Request for <br />Determination of Comparable Housing with the City of Eugene. <br /> <br />2. Ten days following the Request for Determination of Comparable Housing <br />request, the Hearing Officer shall hold a hearing on the request. The <br />determination of the Hearings Officer shall be final. <br /> <br />REQUIRED INFORMATION <br /> <br />The following information or material must be submitted with a Request for <br />Determination of Comparable Housing: <br /> <br />1. Name of condominium or cooperative to be converted: <br /> <br />2. Date of filing request for determination: <br /> <br />3. Name, address, and telephone number of special category tenant or <br />Applicant with whom the dispute is with: <br /> <br />4. Please attach a copy of the completed Comparable Housing Check Sheet <br />which specifically relates to this request. <br /> <br />5.
