' <br /> <br />Appendix B-- <br />Reporting Forms and Suggested Worksheet <br /> <br />Pages B-1 and B-2 show the electronic reporting forms substantially as they will appear to <br />businesses using the reporting software supplied by the City. Complete user instructions will <br />accompany the software when it is sent to reporting facilities. <br /> <br />On page B-3 is the certification form that is to be executed by each reporting company and <br />submitted to the City along with the electronic materials balance report. Only a paper copy of <br />the certification form is required; it is not a part of the reporting software. <br /> <br />Page B-4 is a suggested worksheet that companies may use to assist them in documenting <br />chemical use and calculation methodology.
