SECTION y: RECORD KEEPING FORMS <br />1. Provide examples of your routine inspections check list or log. The <br />log should include: Date and time of inspection, date and time of corrective <br />action, corrective action taken, name of inspector, and counter-signature of <br />responsible official. <br />~. Provide examples of your unauthorized discharge log. Include: Date <br />and time of any discharge, type of material discharged, amount of material <br />discharged, identify if incident was a reportable or recordable discharge, <br />what agencies were notified, cause of discharge, corrective actions taken, <br />time and date of corrective action, method of disposal of discharged mater- <br />ial, and signature and title of responsible official. <br />SECTION yI: EMERGENCY EQUIPMENT <br />The following information must be provided for each storage facility: <br />I. Indicate the type of emergency equipment available and its loca- <br />tion; access and distance from the storage facility, etc.. <br />NOTE: This section may be coded with the facility storage map - <br />Section 1, Part C, far larger facilities. <br />~. Provide a brief description of any testing or maintenance programs <br />for the available emergency equipment. <br />3. Indicate that UFC Standard 79-3 hazard identification placarding is <br />provided far each storage area. <br />4. Indicate that substances in any storage containers are adequately <br />identified. <br />5. Indicate that emergency response procedures are pasted i n a canspi - <br />cuous location near each storage facility. <br />SECTION yII: WASTE HANDLING PROCEDURES <br />List all procedures used at each facilit for the proper disposal of all <br />hazardous wastes. Provide a brief explanation of the method used, when appro- <br />priate. Examples of waste handling procedures include: Disposal at an <br />approved hazardous waste disposal facility, direct discharge to santitary <br />sewer, recycle, pre-treatment discharge to sanitary sewer, etc. <br />Note; A permit may be required from one or more agencies before imple- <br />menting certain disposal methods. Contact the appropriate agency far mare <br />information. <br />APPENDIX vI-A <br />HAZARDOUS MATERIALS CLASSIFICATIONS <br />I. INTENT. This appendix provides information, explanations and examples <br />to illustrate and clarify the hazard categories contained in Division II of <br />Article 80. The hazard categories are based upon the Cade of Federal Regu- <br />Ordinance - 84 <br />
