If <br />this option is selected, a $10.00 service fee will be required before certification is <br />validated and a card issued. Failure to pay the certification fee by the due date can <br />result in revocation of certification. <br /> <br />Standard 1 - The renewal application fee may be reduced to $10.00 if the <br />following conditions are met: <br />1. Has lived in the same area for the past year. <br />2. Has been continuously employed by local licensed companies in the <br />past year, and can substantiate that fact. <br />3.
