<br />Exempt Employee Benefit Summary Recommended for Police Auditor- Attachment C <br />Health Insurance Choice of two plans: <br />City Health Plan - City Managed Care <br />Preferred Provider Plan – Point of Service <br />Organization (PPO) (POS) <br />Payroll Deduction Premium Contribution is 8% of <br />premium based on tiered rate <br />Life and AD&D Insurance City-paid benefit <br />One-times annual salary for life insurance to a <br />maximum of $250,000 <br />Additional one-times annual salary to a maximum <br />of $250,000 for AD&D if death is due to an <br />accident <br />Supplemental Portable Term Life and Optional coverage is available for purchase <br />AD&D Insurance <br />Long Term Disability City-paid benefit <br />60% of salary ($3,000 month maximum) paid after <br />90 day waiting period <br />PERS/OPSRP Retirement Plan Employee contribution of 6% of salary paid by the <br />City of Eugene.
