INDUSTRIAL PRETREATMENT PROGRAM FEE SCHEDULE <br />Fee <br />Annual discharge permit: <br />Annual discharge permit fee is a nonrefundable fee as established by <br />Oregon Administrative Rule 340 - 045 -0075: Permit Fee Schedule Table <br />70E (Annual Pretreatment Fees — Significant Industrial User) and shall <br />be paid to the City by each permitted Significant Industrial User (SIU). <br />The fee shall be due annually on August 1 and will be applied to each <br />SIU specified in the City's annual DEQ Pretreatment Report for the <br />previous calendar year. <br />Mobile waste hauler initial application and permit .............. ............................... $250.00 <br />(Up to two year term) <br />Mobile waste hauler biannual permit renewal ..................... ............................... $ 75.00 <br />(Must be submitted at least 90 days prior to expiration of current term) <br />Application to discharge contaminated groundwater .......... ............................... $125.00 <br />Application to discharge trucked industrial waste ............... ............................... $125.00 <br />Appeal from imposition of administrative penalty .............. ..:............................ $ 75.00 <br />(To be refunded if appellant prevails on appeal) <br />Fee for recovering the City's costs involved in monitoring and resampling permittees who have <br />exceeded a permit limit shall be computed on the basis of the hourly wage then in effect for the <br />personnel performing the service, plus 10% for administrative overhead, together with laboratory <br />costs and expenses. <br />Application fees are non - refundable. <br />The above fees do not apply to City owned or operated vehicles. <br />Exhibit A to Administrative Order No. 58-11-04-F <br />
