Report all bodily irjjury or property damage to members of the public to Risk <br />Services within 7Aours. Reports may be made by phone (541 -682- 5665), but <br />must be followed by a written report as soon as possible. <br />B. Written reports must include the following information (incident report forms are <br />available on Risk Services intranet website, link below): <br />1. Date, time, and location of incident or loss <br />2. Nature of the incident or loss <br />3. Name(s) of involved third party <br />4. Property damage description and /or photographs <br />5. Bodily injury description and /or photographs <br />i <br />
