You must <br />write your full legal name, home address, date of birth, and <br />signature. <br /> <br />New laws require you to give your Oregon Driver's License/ <br />ID number, if you have one. If you do not have an Oregon <br />Driver's License/ID number, you must give the last four <br />numbers of your Social Security number. If you do not have <br />either of these and you are registering by mail, you must <br />mail in a copy of one of the following: <br /> <br />~photo identification (something with your picture, name and <br />address) <br /> <br />~a recent paycheck stub <br /> <br />~a recent utility bill <br /> <br />~a recent bank statement <br /> <br />-+a recent government document (e.g., Oregon Health Plan <br />medical card) <br /> <br />~proof of eligibility under the Uniformed and Overseas <br />Citizens Absentee Voting Act (UOCAVA) or the Voting <br />Accessibility for the Elderly and Handicapped Act (VAEH) <br />
