For the May 2006 Primary Election, you must register to vote by April 25, 2006. You must write your full legal name, home address, date of birth, and signature. New laws require you to give your Oregon Driver's License/ ID number, if you have one. If you do not have an Oregon Driver's License/ID number, you must give the last four numbers of your Social Security number. If you do not have either of these and you are registering by mail, you must mail in a copy of one of the following: ~photo identification (something with your picture, name and address) ~a recent paycheck stub ~a recent utility bill ~a recent bank statement -+a recent government document (e.g., Oregon Health Plan medical card) ~proof of eligibility under the Uniformed and Overseas Citizens Absentee Voting Act (UOCAVA) or the Voting Accessibility for the Elderly and Handicapped Act (VAEH)
