<br />2. The Applicant must prepare and give to each tenant, a copy of a proposed <br />Tenant Assistance Plan. The Tenant Assistance Plan document describes <br />ways the Applicant will assist the tenant who does not buy his/her rental unit. <br /> <br />3. An affidavit must be included with the application stating that: <br /> <br />a. The Applicant has signed receipts from each tenant that they have <br />received a Notice of Planned Condominium Conversion and Tenant <br />Assistance Plan contract; <br /> <br />b. The Applicant will keep all signed receipts on file in this city and subject to <br />inspection for a period of three years; <br /> <br />c. The Applicant has provided the Informational Brochure on condominium <br />conversion provided by the Community Development Division of the <br />Planning & Development Department, City of Eugene to all the tenants; <br /> <br />d. No evictions without cause have occurred between the date of Notice of <br />Planned Condominium Conversion and the date of the permit application; <br />and <br /> <br />e. The Applicant has provided the Tenant Survey Report Questionnaire to <br />each affected tenant. <br /> <br />4. Proof of funding of a replacement reserve account which shall conform with <br />the requirement of the Eugene Code and Administrative Rules. <br /> <br />5. The following additional information must be submitted with the application: <br /> <br />a. Any information submitted to the State Real Estate Commissioner <br />pertaining to the condominium conversion of the building( s) under state <br />law. <br /> <br />b. A rental history report prepared on a form provided by the Community <br />Development Division of the Planning & Development Department, City of <br />Eugene. <br /> <br />c. An executed Tenant Assistance Plan contract that complies with Section <br />2.1070(g) of the Eugene Code, unless all affected tenants have executed <br />binding offers to purchase their units or shares or have permanently <br />relocated at the time of permit application. <br /> <br />d.
