Reporting Exemption Request Form <br />(for Businesses Currently Required to Report Hazardous Substances <br />Under the Eugene Toxics Right -to -Know Program) <br />In compliance with ORS 453.370(8) the City of Eugene has established a procedure for <br />facilities to be exempted from all or part of the Eugene Toxics Right -to -Know Program if <br />they can demonstrate that their operations pose little or no risk to public health or the <br />environment. This procedure can be found on pages 9 and 10 of the program's Hazardous <br />Substance Tracking Instructions. A Reporting Exemption Request Form is what a facility <br />must submit along with a nonrefundable filing fee of $150.001, if said facility believes it has <br />evidence that its operations pose little or no risk to public health or the environment. <br />This form is due to the City of Eugene by July 31st, prior to the calendar year the exemption <br />is being requested for. If granted, the exemption is valid for three (3) calendar years <br />(January 1st through December 31St) after the date of approval. <br />1 The $150.00 filing fee is due and payable the first time a facility requests an exemption. <br />-10- <br />
