Instructions: Use this grid to record your overall ratings from each of the performance <br />categories on the previous pages by placing an X in the appropriate column. Summarize your <br />overall performance rating for the City Manager by placing a 1, 2, 3, 4 or 5 on the line provided <br />at the bottom of the grid. Once again, note that the overall rating for the entire evaluation is <br />subjective for each rater as one rater may decide that different performance categories carry more <br />or less weight toward the overall performance rating than another rater.
