The annual license fee shall be calculated by June 30 of each year and paid by the <br />licensee between July 1 and July 15 of each year. If an audit reflects the licensee has undercharged <br />its customers, the annual license fee shall be recalculated based on the gross collection receipts <br />that should have been charged by the licensee. Any deficiency between the annual license fee <br />previously paid by the licensee and the amount determined due by the audit shall be paid by the <br />licensee within thirty days of notification by the City of the amount due. <br /> <br />3. Deliberate or grossly negligent misrepresentation of gross collection receipts, <br />tipping fees or Lane County system benefit fees by a licensee constitutes a knowing or intentional <br />violation of these Rules and constitutes cause for revocation of the license. <br /> <br />4. License fees for transferred, terminated or forfeited licenses are non-refundable. <br /> <br /> <br />R-3.250-O. Licensee Requirements - Records Maintained. <br /> <br />1. Each licensee shall maintain records for each customer showing: <br /> <br />a. Address of service recipient; <br /> <br />b. Dates of service; <br /> <br />c. Types of service provided, including, but not limited to, solid waste <br />collection, curbside recycling, yard debris collection, organics collection, commercial food <br />waste collection and additional services, and the frequency of each collection service; <br /> <br />d. Rates charged for service; <br /> <br />e. Rebates, discounts or credits provided to customers; and <br /> <br />f. Donated services provided to customers. <br /> <br />2. Each licensee shall maintain records showing the basis for: <br /> <br />a. The rates charged and rebates, discounts, credits and donated service <br />provided to its customers; <br /> <br />b. The license fees paid to the City; <br /> <br />c. Allocating expenses between the licensee’s customer accounts inside the <br />City limits and those outside the City limits;
