That no rent increase since the notice <br />of planned conversion for any affected tenant has <br />occurred except in a proportional amount equiva-~ <br />lent to th.e rise in the All Ttems Portland Consumer <br />price Index since the date of the last rent in- <br />crease for the unit in which that tenant resides. <br />~_d~ .Provision of informational documents. The <br />following documents shall be provided with the applica- <br />tion for a preliminary permit for condominium conversion: <br />1. Information to state. All issued condo- <br />°min~.um i.nstruments.~ <br />2. building history. A building history re- <br />part showing, if such information. is reasonably <br />available, the date of construction of all common <br />elements and components thereof of the building, a <br />statement of the major uses of such building since <br />construction, the date and description of each major <br />repair of any common elements since the date of con- <br />struction, the date and description of each major <br />renovation of any element since the date of con- <br />struction, and the names and addresses of the current <br />owners~of all improvements and the underlying land <br />of the building proposed to be converted. The de- <br />finition of "major repair" and "major renovation" <br />shall be set under administrative rules issued here- <br />under. <br />3. Rental history... ~?. rental. history report <br />,.,,,, <br />detailing, if such information is reasonably avail- <br />abler the size and square footage of the units in <br />each building, the current or last ~'ental rate, the <br />names and addresses of each present and each <br />affected tenant, the monthly rental rates for the <br />preceding three years for each unit,~the monthly <br />.vacancy over the preceding three years, the number <br />of evictions over the preceding three years, the <br />name of each special category tenant presently <br />residing in the project, and the number and type <br />of special category tenants over the preceding <br />three years.
