City Manager Feedback Survey-Executives <br />The Eugene City Council is inviting members of the community and the City of Eugene organization to contribute <br />feedback to their annual performance review for Eugene City Manager Jon Ruiz. <br />As a direct report to the City Manager, your feedback will be invaluable. We ask you to be a contributor by taking a <br />few moments to fill out this survey. Your candid input is valued and respected. Please respond to the following <br />sections of the survey and then "submit" it. The responses will be collected by Denise Smith in the Human Resources <br />Department and summarized for the City Council. <br />The Council will use this summary in order to inform their overall evaluation of City Manager Ruiz's performance this <br />past year and to set future goals with him for the coming year. <br />If you have any questions regarding completion of this survey, please contact Denise Smith at X5731 or <br />denise.d.smith@ci.eugene.or.us. <br />Once again, we appreciate your taking time to contribute to this important process. <br />2.
