'EUGENE' <br />City of Eugene <br />Administrative Policy Manual <br />Date Updated <br />14.3 <br />August 1, 2011 <br />Policy Title: Reporting Property Losses <br />Purpose: To outline the procedure for reporting property <br />Scope: Applies to all City employees. <br />Policy Statement: Risk Services is responsible for the ident��cat�4 1" 1 evaluation, <br />I 1e <br />treatment, and monitoring of the City's operati (bks mclu g p ,,and other <br />�� u..0 <br />miscellaneous insured losses. Risk Services re ies gthe department avisions to <br />report all property losses., <br />Policy Procedure: All damage, theft ar, ss of City property, including loss of cash, <br />must be reported to the responsib�(etsupervisor and Risk Services as soon as possible. <br />Risk Services will work with the departmAffi <br />nc e ivision to dete�ranine if there.is coverage <br />under the City's property insurapolicy or if recovery from the responsible party (or <br />their insurance company) is possib k <br />. <br />A. All prope age and lass must b,0 "`reported to the responsible supervisor and <br />Risk Servic� „gn as possible following an occurrence. Reports may be <br />made by ph e, -a followed by a written report (memo or email). <br />Losses of cas inidtli�bks, credit card receipts and currency, must also be <br />reported to the tnance Diyision, Treasury Officer. <br />B. Written reports musDnclude the following information: <br />1. Date, time, and location of loss <br />2. Nature of loss (vandalism, fire, traffic accident, theft, etc.) <br />3. Description and /or photographs of damaged property <br />4. Documentation that the Police Department was contacted, if appropriate. <br />Include the EPD case number if possible. <br />5. Itemization of repair /replacement costs. Examples of such costs include <br />materials, labor, and equipment use costs and miscellaneous overhead <br />costs. <br />6. If possible, the name and address of the individual who caused the <br />damage and his /her insurance company's name and address <br />7. Name of the reporting supervisor /employee. <br />
