After signature verification (see 2(d) above), the candidate wil~ submit to the City Recoraer's Office no ~ater than 5:00 p.m., March 2, 2006, the following forms: a) Candidate must file with the City Recorder a "Perfected ?etNon." A perfected petition is identified as such by checking the box on the form SEL 120 - FLUNG OF CANDIDACY FOR NONPART)SAN NOMINATION. b) Candidate must submit SEL 121 - PETITION FOR NONPARTISAN OR ~NDEPENDENT NOMiNATiON, with a minimum of 25 valid signatures verified by Lane County Elections. Financia Reporting Requirements All candidates who expect to raise or spend more than $300.00 during the campaign are required by Oregon State law to fi~e a "Candidate's Statement of Orgamzation" (SEL 220) and financia~ reports~ The form must be filed within three (3) days after collecting funds or making an expenditure, All financial reports must be filed with the City Recorder's Office The level of reporting required will be determined by the amount raised or spent. Statement of Economic Interest State Reporting Requirements All persons holding office in the City of Eugene, including the Eugene Water & Electric Board, are required to file annual Statements of Economic Interest with the Oregon Government Standards and Practices Commission. These reports must be filed by April 15 every year of the incumbency of the elected official, information will be requested concerning sources of income, property, businesS interests and gifts related to the office. Civil penaltie~ may be imposed for failure to file, or for insufficient information. For more information on this reporting requirement, please contact the City Recorder's Office.
