Upon termination of ser- <br />vices to any subscriber, the Grantee shall remove promptly all its above <br />ground eternal facilities and equipment from the premises of the subscriber <br />at the owner's written request. <br />~3} Complaint Procedures and_1nqu,~,,ri es. <br />~a} Grantee shall maintain an office in the city of Eugene, <br />which shall be open during ali the usual business hours with its telephone <br />listed in directories of the telephone company serving the franchise terri- <br />tory, and be so operated that complaints and requests for repairs or adjust- <br />ments may be received at any time, day or night, seven days a week. The phone <br />number and address of this office shall be furnished to each subscriber by <br />the Grantee. <br />~b~ Except as provided in paragraph ~c~ of this subsection, <br />original records including service records pertaining to complaints received <br />by the Grantee, and of the office procedures followed to satisfy those com- <br />plaints shall be maintained by the Grantee for a period of~not less than 3 <br />years in Lane County, Dregon, and made available for inspection by the com- <br />mission vn reasonable notice to the Grantee. This record shall be considered <br />by the commission in evaluating Grantee's system.
