<br />CITY OF EUGENE <br /> <br />Revised Solid Waste & Recycling Collection License <br /> <br />Northwest Resource Recycling~ Inc. <br /> <br />1. Northwest Resource Recycling, Inc. (Licensee) is authorized by this license to <br />collect industrial and commercial solid waste only within the City of Eugene (City), providing the <br />solid waste is transported to a materials recovery facility at 2034 West 2nd Avenue, Eugene, <br />Oregon, all materials designated as principal recyclable materials for the Lane County wasteshed <br />are removed therefrom, the remaining solid waste is thereafter transported to a state approved <br />landfill or waste transfer station, and this activity results in a mininum recovery of 40 % for <br />accounts collected with the Eugene City limits for the period from 1995 to 1999, and a minimum <br />recovery of 60 % for accounts collected within the Eugene City limits for the year 2000. <br /> <br />2. In addition to compliance with the specific terms imposed herein, Licensee shall, <br />throughout the license term, comply with all local and state land use, building code and <br />environmental law requirements and secure and provide City with proof of any permits or licenses <br />required thereby. A violation by Licensee of any of those requirements or laws shall constitute <br />a violation of this license. <br /> <br />3. Licensee shall be subject to and agrees to comply with City's solid waste and <br />recycling collection Code and Rule provisions, specifically including the minimum/maximum rate <br />schedule and the requirement to provide financial records and tax returns when requested to do <br />so by City for rate studies and license fee determination. <br /> <br />4. To substantiate the reduction in waste load, the Licensee shall submit to City, on <br />a monthly basis: <br /> <br />4.1 The total number of its Eugene collection accounts; <br /> <br />4.2 A breakdown of those Eugene accounts by container size; <br /> <br />4.3 The total amount of material collected, by tons; <br /> <br />4.4 The total amount of solid waste disposed, by tons; and <br /> <br />4.5 The total amount of recyclables recovered, by tons. <br /> <br />These items shall be verified by account records, weight slips and sales receipts. The monthly <br />reports and verification shall be submitted one month from the date this license is issued and every <br />month thereafter. A report thereof shall be delivered to City by the 10th day of each month. <br /> <br />5. Failure to submit a report as required in paragraph 4 above shall result in a penalty <br /> <br />Exhibit A - License - 1 <br /> <br />01117/96 <br />
