Employees are responsible for notifying their supervisor as soon as possible if they <br />become ill and must leave work prior to the end of their shift. <br />Proof of Illness /Return to Work <br />Verification of illness by a doctor's certificate may be required by employees' <br />supervisors prior to authorizing sick leave. A doctor's statement, satisfactory to the <br />City's contracted medical services provider, may be requested by supervisors to <br />determine the status of employees' health and/or physical limitations. It may be used to <br />reassign employees to limited -duty in accordance with the doctor's recommendations, <br />or to help supervisors with absenteeism control. It may not be used as an enforcement <br />tool by itself or as a means of harassment. Employees <br />must give immediate notice of the injure ess to their supervisor and, if requested, <br />provide doctor's verification that is satisfa Wy.ta the City. <br />Return to Work "111111 Ii u, f <br />If an employee has the work place for more than 30 days due to a or non - <br />work related illness ( intu �11'� <br />f mployee has a physical or mental condition <br />which may impair th T ' 06yee ity to perform the regular duties of their job, the <br />employee will be required to provide a release from their attending physician before <br />returning to work. <br />The supervisor should initiate this process by writing a letter to the attending physician <br />asking the physician to authorize the employee to return to their regular assignment. <br />The letter should include background information on the employee's regular duties, <br />samples of work assignments and a list of the essential job functions. Sample letters <br />are available from Human Resources or Risk Services. <br />Sworn police officers and firefighters who are required to take a physical agility test will <br />be retested before returning to work if they have been out of the work place for 6 <br />months or more, or have missed a regular testing period. <br />
