II�uA� <br />The City is also committed to having a proactivelantl�effective safety prram with <br />support of both management and employees. <br />Policy Procedure: Supervisors and ployees are responsible for employee safety <br />and accident prevention, including:usmgsafa methods, prae#ices, and equipment <br />relative to the job being perforr�eda Employees are also res�iansible for reporting <br />accidents, faulty equipment, uii A, practides of fellow employees, and/or unsafe <br />conditions of work areas to their suoerviso s. <br />Safety Committees and Safety <br />The purpose of safety committees and safe't Iffibetings is to bring employees and <br />management tog6l in a cooperative effo , o promote safety and health, and to make <br />continuous impro � � � gR,,the City's safety and health programs. <br />Safety committees rfl�y �#.1 61110'to assist departments in improving the <br />effectiveness of their ppgrams. Committees should function in a staff or advisory <br />capacity only. Responsibility and accountability for creating and maintaining a safe <br />environment and review.f employee accidents for corrective action, including work <br />evaluation and discipline ponsiderations, will remain the responsibility of supervisors. <br />Safety Inspections <br />Each department or division Safety Committee is required to establish procedures for <br />conducting safety inspections of all facilities (including mobile work sites) and <br />equipment at least quarterly. The results of all inspections must be documented and <br />kept on file with the department or division. <br />Accident Reporting <br />When an accident or injury occurs, it must be reported immediately to the employee's <br />supervisor. All lost -time injuries must be investigated and documented and the <br />information provided to Risk Services. It is the responsibility of Risk Services to <br />
